use consistent pattern for file inclusion guards
* marker.h, ls-utils.h, oct-hdf5.h, pt-colon.h, pt-fcn-handle.h:
Use consistent pattern for file inclusion guards. Ensure that symbols
on the "#if ! defined" and "#define" lines match.
